Veterinary Checkup
A veterinary checkup must be made with your veterinarian within 5 days after receiving the puppy, (excluding Sunday & Holidays), to determine that it is in good health. If it is not given a clean bill of health, Sellers must be notified by phone or written notice within one week of checkup.

If you cannot get your puppy checked by a licensed veterinarian within 5 days, Sellers must be notified immediately. Any communicable or contagious diseases contracted after the 5 day period is not covered by this guarantee. This guarantee does not cover a rare occasional congenital defect which may occur but is not life threatening, ie: food allergies, knee, eye or skin problem, etc.

Should the puppy develop a life threatening defect of the heart, liver, or kidney, and require euthanization before the age of one (1) year, Buyer will receive a replacement puppy from the next available litter. Buyer assumes full responsibility for all medical costs and care. NO reimbursements will be made on these costs. Proof of veterinary care, euthanization, diagnostic reports, all health records from your vet, autopsy report, and a written report of the dog’s health problem shall be furnished to the Sellers before a replacement puppy is given.

Please sign one copy of this agreement and return with a copy of your first Vet Receipt in the postage paid envelope provided. Failure to return will nullify and void agreement.
